Disclosure policy

This Disclosure policy outlines how Promise Comfort Care LLC is responsible for ensuring equal access to company services is provided to persons with Limited English Proficiency. This policy pertains to the language needs of Promise Comfort Care LLC clients/consumers. It does not apply to employees who, to perform their duties, must be fluent in the English language. Promise Comfort Care LLC will take steps to ensure everyone has meaningful access and an equal opportunity to participate in its services, programs, and other benefits. Promise Comfort Care LLC recognizes that while doing business, our company collects personal health information about our clients and our employees. We respect and value our clients and employees’ trust in our company to ensure that their information is kept safe and secure from inappropriate disclosure or use. Promise Comfort Care LLC is committed to protecting the privacy of individual health information in compliance with (HIPAA) and the regulations promulgated there under. These policies and procedures apply to protected health information created, acquired, or maintained during the operations of Promise Comfort Care LLC.   Promise Comfort Care LLC makes every effort to ensure that personal health information is accurate and current.  
Clients have a right to access their personal health information. Clients can review the paper record that is located in their homes. Electronic records can be accessed by contacting our office. A client will be asked to sign a release and show photo identification before releasing any health record.
Following HIPAA and its regulations, our company will obtain a personal allowance to use or disclose protected health information. Generally, designated covered components do not need to obtain an individual’s authorization when using and disclosing protected health information for routine purposes (e.g., treatment, payment, health care operations) or other limited purposes.  The client can withdraw their consent to share personal health information anytime.  Client information may be accessed by regulatory authorities to defend a legal issue. If an unusual request for your information is received, we will contact you for permission to release such information. We may advise you if such a release is inappropriate. You may withdraw your consent for using or disclosing your personal information, and we will explain the ramifications of that decision and the process. It is the policy and commitment of Promise Comfort Care LLC that it does not discriminate based on race, age, color, sex, national origin, physical or mental disability, or religion. All services provided by Promise Comfort Care LLC will be delivered following the Civil Rights Act of 1964 and in compliance with Chapters 51 and 611 under the State of Pennsylvania’s Department of Health code for home care, agencies/registries.  Our agency seeks to hire the most qualified and talented direct care workers to assist clients/consumers to live as independently as possible while performing their activities of daily living.
